Effective Date: January 1, 2026. This comprehensive document serves as the official informed consent and privacy policy for patients receiving online consultations, telehealth services, and remote herbal guidance from Flatirons Acupuncture, managed by Erika Weich, L.Ac., MSOM, based in Boulder, Colorado. Please review this information carefully before proceeding with virtual consultation programs.
1. INFORMED CONSENT FOR TELEHEALTH
Telehealth refers to the practice of providing healthcare, clinical evaluation, and therapeutic consultation using secure audio-visual telecommunications technology. By booking an online consultation, you understand and acknowledge that: (a) Information transmitted may include medical records, live visual assessments, and personal health narratives. (b) There are inherent limitations to remote assessments compared to in-person Traditional Chinese Medicine (TCM) evaluations, specifically the inability to physically palpate acupuncture meridians or take manual pulses. (c) Alternative methods, including seeking local in-person acupuncture treatments in Boulder, CO or your residential area, remain available to you at any time.
2. TELEHEALTH SCOPE, SECURITY & PRIVACY AGREEMENT
Telehealth involves providing herbal consultations via secure video technology. While effective, it has limitations compared to office visits as it lacks physical exams and hands-on pulse or tongue diagnosis. By proceeding, you consent to receive remote care and acknowledge that video links and charts are managed through encrypted, HIPAA-compliant platforms to ensure data security. Please refer to our full Notice of Privacy Practices and Privacy Policy for details on your rights. This service is not for medical emergencies - call 911 or go to your nearest ER. Authorized staff access records only for treatment logistics, such as formula adjustments or billing.
3. HERBAL FORMULA SHIPPING & RESPONSIBILITIES
Our new specialized online consultation feature permits custom Traditional Chinese Medicine herbal formulas and premium essential oils to be prepared and shipped to patients nationwide. You acknowledge that these customized herbal formulas are configured explicitly based upon the diagnostic intake provided during your virtual assessment. It is your strict responsibility to report any sudden physiological changes, potential prescription conflicts, or pregnancy statuses prior to compounding or consuming shipped formulas. Flatirons Acupuncture is not liable for mail transit delays once shipments leave our Boulder distribution center.
